Thank you for considering our church team ministry.
The church team leader should contact the U.S. MAPS department to establish a team project, dates and team skills that are needed. The team leader is responsible for the U. S. Maps Trip Registration form and Insurance List form.
An Individual or Team Application Form must be completed by each person and returned to the U.S. MAPS office at least 2 week prior to leaving for your assignment.
Each person of the team must sign and return the Assumption of Risk, Code of Conduct and Guarantee Trust Life Insurance form prior to the team’s departure. The insurance is $3.00 per day per member, from the day you leave until the day you return. All these forms must be in U.S. MAPS office at least 2 weeks before departure for us to give World Ministries Giving Credit to the sending church.
The team leader should communicate with the U.S. MAPS office to be sure all paperwork is processed.
We now offer membership to our network of volunteers. The individuals complete the Membership Network Application and pay the $10.00 fee. (active for two years). This enables members to go on Missions trips without filling out the individual paper work for two years. We in turn issue a membership card that can be used for future trips.
